Notice of Kids Care Dental & Orthodontics Practice Relocation

Dear Parent/Guardian of KCD&O:

We are pleased to share some exciting news regarding our Pleasanton Kids Care Dental & Orthodontics practice. Due to our consistent growth and success at our Pleasanton location, we have found ourselves in need of a larger space.

Therefore, we are thrilled to announce that we will be relocating to a new state-of-the-art facility in Livermore, just 3.9 miles away. Our new address will be: 3000 W. Jack London Blvd, Space A-1 Livermore, CA 94551. This relocation will be effective starting from August 1st, 2023. We are confident that this move will allow us to better serve our patients and provide an improved experience for everyone.

We know that our patients love to see Dr. Kapoor, Dr. Dal Porto & Dr. Lee for their oral care needs and all doctors will continue to treat their patients at our new Livermore location.

We understand the decision to relocate our practice may affect our patients and families. We welcome the opportunity to continue to serve you at this new location, and we have solutions in place to assist in the transition. Our goal is to ensure minimal disruption and continuity of care. Below are the options we can provide that may meet that goal. Please contact us at 916-570-1222 to discuss your options. 

1. You may continue your care at any one of our neighboring practices. Each of our practices offers the same patient-centered care you have come to know and love. We encourage you to transfer care to our Livermore location, which is located at:
   • If you prefer a different Kids Care Dental & Orthodontics office, please visit kidscaredental.com/locations for a full list of locations.

    

2. You may transfer your care to a new provider of your choosing, outside of a Kids Care practice. With your authorization, copies of the dental records will be shared with you or directly with the new provider.
   • Request an “authorization to transfer” form via phone during regular business hours or email us at [email protected]. You must have a copy of the authorization form completed and signed before we can release the records.

    

3. If you have an orthodontic appliance (i.e. braces), you may elect to remove it. While we don’t recommend early removal of appliances due to continuity of care, you may elect to discontinue your orthodontic treatment all together.
